For Hospitals
Care Finder-Pro connects hospitals with an online network of more than 70,000 providers.
Case providers have access to detailed provider information and can quickly connect
with providers to secure patient placement. Results include lowered length of stay
and time per discharge plus increased patient and employee satisfaction.
The Care Finder-Pro system securely stores and manages all patient documentation
electronically for distribution when requested. The result is a transitional care
workflow that is quick, secure and accurate. Hospital staff now can spend more time
with clinical responsibilities and less time with clerical paperwork.
Closing the Case and Management Reports
Information from the queried facilities are now handled electronically assuring
an accurate and timely response. Once the patient has selected the post-acute provider,
the Planner “clicks" or sends a one-page fax, and the Care Finder-Pro™ system
notifies all the providers that the case is closed. It also notifies the physician
where her/his patient now resides and prepares management reports on the provider
and planner’s activity. The results are improved hospital/provider relationships,
better physician communication, and objective management reporting.
For Home Care Providers and Facilities
TLC believes in building the most comprehensive database of providers as possible,
which is why we include all registered providers in our database free of charge.

provider survey to introduce consumers and hospital workers to your services.
For Consumers, Discharge at Admission
A consumer friendly version of the search tool is available to all patients via
TLC's family programs. on-line search tools. Care Interpreter helps users determine
the appropriate type of care they need. Facility Finder identifies facilities that
meet the family's quality of care needs and quality of life wants. Home Care Finder
helps identify resources for patients who are in need of care, but prefer to return
home.
Through a series of questions, the online systems help identify options that match
clinical needs with lifestyle wants. Together these tools increase the family's
post acute involvement and in turn, patient satisfaction.

be accessed directly through TLC's Family Programs.
