Total Living Choices Partners with Los Angeles County Healthy Department to Streamline the Patient Discharge Process

Seattle, Wash. (Sept. 15, 2010) – Los Angeles County Health Department has signed a partnership agreement with Total Living Choices, a premier health-care technology company that offers software solutions to help patients, families and health-care providers coordinate post-acute care.

This partnership follows the recent contract between TLC and Rancho Los Amigos National Rehabilitation Center. Like Rancho Los Amigos, Los Angeles County Health Department will now have access to TLC’s Care Finder-Pro® discharge networking software. Care Finder-Pro, the company’s flagship product, is installed at hospitals throughout the United States. Hospital personnel use the product to quickly and cost-effectively research available providers when discharging patients. Care Finder-Pro securely stores and manages all patient documentation electronically and can be customized to each hospital's requirements. Physicians are notified of patients' facility choices and will receive management reports on provider and planner activity. 

Director of Los Angeles County Health Department Dr. Jonathan E. Fielding believes that this partnership will be beneficial to patients, families and staff alike. “We recognize what a difficult process it can be for patients or their families to determine the best post-acute care options. Through our partnership with TLC and the addition of Care Finder-Pro, we hope to ease this burden and help patients and their families when trying to determine the best transitional care choice based on their quality of life wants and quality of care needs,” Dr. Fielding said.

Total Living Choices CEO Ted Tanase said that together, TLC and Los Angeles County Health Department will educate patients on available providers in their time of need. “With more than 68,000 post-acute providers nationwide, we have the most comprehensive database on the market,” Tanase said. “We believe patients should have access to every available resource.
