Total Living Choices and Missouri Health Care Association Partner to Educate
Patients and Families
Seattle, Wash. (Aug 6, 2010) – Total Living Choices, a premier health-care technology
company, announced a partnership with Missouri Health Care Association. TLC offers
software solutions to help patients, families and health-care providers coordinate
care transitions for patients following discharge.
Through the partnership, MHCA members now have access to TLC’s online Family Programs:
Care InterpreterÒ, Facility FinderÔ and Home Care FinderÔ. This suite of programs
provides web-based solutions to match patients’ quality of care needs and quality
of life wants while researching post-acute care options.
MHCA is the largest long-term care association in Missouri, with more than 300 licensed
care providers throughout the state. Total Living Choices’ hospital and consumer
resources connect patients and families with appropriate care providers
nationwide.
In addition to TLC’s Family Programs, Care Finder-ProÒ, the company’s flagship product,
is installed at hospitals throughout the United States. Hospital personnel use the
product to quickly and cost-effectively research available providers when discharging
patients.

model that requires providers to pay a fee to be included in our database.”
