Total Living Choices and the California Association of Health Facilities Partner
to Educate Patients and Families
Seattle, Wash. (May 24, 2010) – Total Living Choices, a premier health-care
technology company connecting patients, families and health-care providers to coordinate
care transitions and locate the most effective living solutions, announced a partnership
with the California Association of Health Facilities.
The CAHF is a non-profit association that represents more than 1,250 long-term care
providers throughout California. Total Living Choices’ hospital and consumer resources
connect patients and families with appropriate care providers nationwide.
Through the partnership, CAHF members now have access to TLC’s online Family Programs:
Care Interpreter, Facility Finder and Home Care Finder. Care Interpreter helps patients
determine the level of care required, Facility Finder helps identify a provider
that matches a patients’ clinical needs and personal wants and Home Care Finder
helps patients select an in-home service provider.
In addition to TLC’s Family Programs, Care Finder-Pro, the company’s flagship product,
is installed at hospitals throughout the United States. Hospital personnel use the
product to quickly and cost-effectively research available providers when discharging patients.
